Printing Lab Form

Before printing a lab form and sending it to the lab:
 

  • Clinic panel should collect all screening samples from the patient and ensure that they are securely packaged and labeled to prevent any errors or confusion during transit. 
  • It is crucial to ensure that all necessary patient information is accurately recorded and saved. This includes completing all relevant history questions, Dass questions, occupation background, and mammogram questions.
  • Accurately key in all patient vitals, and save this information.
  • Clinic panel should select the appropriate state, town, and lab in the lab selection, and ensure that all information is correctly saved before proceeding to print the lab form. 

     

After printing the lab form:

  • Once the lab form and samples are ready, the clinic panel can send them to the lab for testing.
  • Please noted clear communication and collaboration between the clinic panel and the lab can help ensure that the samples are tested accurately and efficiently.
